Dear Parents,
Welcome to Foster Elementary School. I look forward to working with you to provide the best possible care for your children while they are at school. In order to do this, I am asking for your help in maintaining a healthy environment for your children as well as the other children in our school.
Please use the following guidelines when determining whether or not to send your child to school if they are not feeling well. If you have any questions about whether or not to send your child to school, please call the clinic. Also, if your child will be absent for any reason, please call the absence reporting line at 281-641-1419 and leave a message.
Please make sure that your child’s Emergency Phone Numbers and Health Inventory are current. Please notify the school of any changes during the school year in writing or in person in the front office.
The Health Inventory enables the school nurse to understand and meet the special health needs of your child. Please list all current medications and health conditions.
ALL MEDICATIONS MUST BE BROUGHT TO THE CLINIC BY THE PARENT OR AN ADULT. NO CHILD IS ALLOWED TO BRING MEDICATION TO SCHOOL.
An Humble Medication Form must be filled out for all medication, prescription or non-prescription. Medication must be supplied in it's original containers and will be kept in the clinic. Prescription medication must have the label from the pharmacy with the child’s name, and will require a Humble Medication Form to be completed by the doctor. These forms are available in the clinic and online on the nurse page of Foster Elementary website. Cough drops are not FDA approved and cannot be given at school per Humble ISD Health Services.
