RankOne Forms for Incoming 7th-12th Grade Students for the 2023-2024 School Year
(Athletics/Cheer/Drill Team/Band)Dear Parents of Incoming 7th through 12th Grade Students,
If your child will be participating in athletics, cheer, drill team, or high school band (Not PE) during the 2023-2024 school year, you will need to complete an Emergency Card, sign the Humble ISD/UIL Rules, and, if playing football, complete the Helmet Form. These forms, along with a current physical on file (dated after February 1, 2023) must be completed prior to being allowed to participate next school year.
The forms may be found online at http://tiny.cc/humblesport or you may use the QR code below. It is recommended to complete the forms as a guest rather than making an account.
2023-2024 Athletic Physical Form & Uploading Instructions for Rank One
The 2023-2024 physical form is now available. Please make sure the physical is received and dated after Feb. 1, 2023. The form must be filled out in its entirety with no blanks on either side. Current 8th graders(Incoming Freshman), please make sure to complete the ECG part of the form.
A new feature this year on RankOne is the ability to upload a completed physical form. If you are using this feature, please complete the Medical History Form which you will find on the link for the physical upload. Additionally, please add your contact information in the event of issues with the physical form. Uploading a physical form does not ensure that your child will be cleared immediately. Athletic trainers will review each uploaded physical form and, if the upload is not approved, a message will be sent to the email attached to the uploaded physical.
If you have turned in a hard copy of the physical form to the athletic trainers/middle school coaches, you do not need to complete the medical history page, only the forms mentioned above.
Additionally, if your child has a medical condition that necessitates the use of medication (asthma, diabetes, seizures, severe allergies, etc.) it is required that they have their medication at every practice or game. Please be prepared to provide necessary medications to their athletic trainer/coach before they will be allowed to participate. Medications may be picked up by the student following the end of the season or school year. This requirement is for the safety of your child as athletic trainers or coaches do not have any emergency medication. Paperwork regarding the medication must be on file with the school clinic.

Calling All Immunization Records!, Due ASAP
CALLING ALL IMMUNIZATION RECORDS!!
As your children receive their middle school immunizations, please remember to bring or send a copy of their updated immunization record to Nurse Brown at the WMS clinic. Many of your children have probably already received these immunizations. For those who still need their updated vaccines, please make these appointments very soon. According to Texas state law, without proof of the required vaccines, your child(ren) will not be able to attend school and may be withdrawn. This is especially important for 6th grade students who are entering 7th grade. Please call Nurse Brown at 281-641-5205 or email at stacy.brown@humbleisd.net if you have any questions.

Help WMS raise money with Box Tops for Education
WMS PARENTS & STAFF:
Box Tops for Education has been around for 25 years, giving families an easy way to help earn money for schools with products that we buy every day. But remember how involved it used to be with all of the cutting, counting and submitting the box tops? It was a huge undertaking - I remember! But now, there's an app for that and it's SO easy! We all buy groceries and shop for household items, right? Even if you have your groceries delivered, you can still use your online receipts!
If you don't already have the app, you can scan the code on the attachment and get it in seconds and start earning for your student's campus. We would really appreciate your participation. Please see the attached flyer and help us earn money for our students at WMS. We have a lofty goal this year of $1500 and we'd LOVE to exceed that! Please help us!

Woodcreek Middle School offers tutoring in all subjects several times a week. Please view the tutoring schedule to make plans for your child to attend one today.
Please note that students must sign up for tutorials with the teacher and get a pass to re-enter the building for tutoring. A student can attend tutorials from any teacher in same grade and subject to get help.

Complete the Woodcreek Cares Feedback Form
One of our goals this year is for each member of our staff to provide ELITE Customer service to you. Our customer service campaign is called Woodcreek C.A.R.E.S (Caught Acting and Responding in Elite Style). We would like you to use the form to let us know about your interactions with teachers, administrators, office professionals, or other staff members on our campus throughout the year.
Please share with us when you have had a positive or negative experience. Your feedback will allow us to celebrate those who are doing well and help us find areas to grow.

Kindergarten through 11th Grade Final Report Cards
Final report cards for students in Kindergarten through 11th grade are now available in Home Access Center (HAC), and will be viewable in HAC through Tuesday, June 27. After June 27, information for the 2022-2023 school year is no longer viewable in HAC because the system has rolled over to the new school year. View instructions for how to view a report card in HAC.
Graduated seniors will receive a final report card via US Mail. Access to HAC is no longer available once a student has graduated. -
2023 Summer School
Summer School for middle schools will be held from 7:30 am to 1:00 p.m. June 6 through June 22.

At the January 10, 2023 Board of Trustees meeting, the 2023-2024 District Calendar was approved. The approved calendar closely resembles this year's calendar to support consistency for students, parents and staff.

Woodcreek Middle School Technology Schedule -
WMS Retest Policy
Students who earn less than a 70 on any summative may retest for up to a 70. Test corrections are not an acceptable form of retest. Summative project grades will follow the same guidelines as traditional assessments. The following must be done by the student within one week of the test being returned.
-Students must not have any missing work.
-Students must schedule and attend a minimum of one tutorial.
-Students must retake exam within one week from the time the graded test is returned to the students.
Please Note: High School credit courses will be limited to one retest per nine weeks. Retest policy is consistent with SCHS. -
